?? Complex Care Assessor – Layola Care (Full-Time)
?? Based in Rochdale
Layola Care are seeking a skilled and compassionate Complex Care Assessor to join our professional, friendly team. This is a vital role supporting individuals with complex physical, behavioural, and mental health needs ??.
This is office hours 9am-5pm but they must take part in the on call rota.
?? About the Role
You will:
• ?? Complete comprehensive care assessments and reviews
• ??? Develop and update individualised care plans
• ?? Liaise closely with families, health professionals, and care teams
• ???? Support staff with guidance, training, and supervision
• ?? Ensure compliance with Care Quality Commission (CQC) standards and Layola Care policies
• ?? Take part in the shared on-call rota
?? About You
We’re looking for someone who is:
• ?? Experienced in health or social care (Senior Carer, Nurse, Team Leader, Care Coordinator)
• ?? Confident assessing complex needs and developing detailed care plans
• ?? Organised, proactive, and an excellent communicator
• ?? Compassionate and committed to person-centred, high-quality care
• ?? A driver with access to a vehicle (essential)
