CARE CO-ORDINATOR
JOB DESCRIPTION
MUST BE WITHIN 25 MINUTES OF KT6 7EL (Wimbledon Office)
PAY: £28,000-£30-000 (Depending on experience) + £100 per on call weekend (£1200 annually)
RESPONSIBLE TO: Deputy Care Manager
JOB PURPOSE: To manage all aspects of future and day-to-day planning, to ensure smooth care service delivery
WORK HOURS: 40 hours per week plus on-call duties as per the office rota (one weekend in four on-call, one evening/morning per week on call)
MAIN DUTIES:
Optimally allocate CareAngels based on skills, availability, experience and client needs to deliver the best quality care and support to each of our clientsOrganise CareAngel rotas to minimise travel between clients and ensure guaranteed hours are allocated, where necessary
Respond efficiently to day-to-day changes in client care needs and CareAngel availability, communicating professionally to all concerned
Accurately record all changes to scheduled visits in a timely manner to ensure clear visibility and full traceability
Maintain accurate and professional written communication
Monitor all relevant aspects of the care management system
Remains hands-on in the field by attending scheduled care visits
Undertake on call duties as per the office colleague rota
ACTIVITIES:

·Future planning of rotas oEnsure rotas are planned 4 weeks in advance oMaintain trackers to identify upcoming capacity constraints and recruitment needs oEnsure guaranteed hour CareAngels have the correct number of hours oComplete Return to Work forms for all colleagues on sick leave ·Welcome new colleagues oMeet and greet new colleagues during their training oBook shadow visits and communicate with new colleagues oAdd new CareAngel availability on OTH ·Other planning tasks oUpdate the Operations Manager on branch care hours capacity oUpdate Recruitment Manager on recruitment needs oUpdate CareAngel availability oManage CareAngel holiday requests oEmail client rotas oNotify CareAngels of finalised rotas oFollow up with clients on service suspend ·On-call duties as per rota |

·Monitor One Touch Health to ensure no missed visits, medications or tasks ·Care Coordination oManage CareAngel sickness and cover unallocated visits oRespond to rota change requests e.g. cancellations, time changes, ad-hoc extra visits oCommunicate and confirm rota changes with clients and CareAngels ·Colleague profile changes oAmend digital tasks and medication on OTH as required ·New client enquiries oTake new client enquiries and complete enquiry form oSend brochures by post / email oBook assessments with Registered Manager and/or Client Advisor oFollow up accordingly on all enquiries as required ·Other oSupport Office with non-urgent tasks e.g. placing orders for PPE, stationery etc, organizing post, add CareAngel contact details to on-call phone oWork in accordance with the policies, procedures and philosophy of Guardian Angel Carers oContribute to a healthy and safe working environment oAny other tasks as delegated by the Business Owners |
