Purpose of Position

 

To monitor, review, maintain and respond to the health and care needs of the resident group.

 

Responsibilities:

·      To ensure that clinical healthcare needs of residents to promote understanding and awareness within the wider staff team of resident’s health and care needs, journey and upcoming interventions. Promote and ensure relevant awareness and attention to needs and task is endorsed within the staff team and to monitor and follow up the daily consistency of their delivery. 

 

·      Reviewing clinical, healthcare and medication policies and procedures and updating as required

 

·      Generating and implementing care plans, in relation to medications and health care procedures

 

·      Supporting, overseeing and reporting on district nurses’ visits and other allied health professional appointments in and out of the home. Disseminating instruction, care and guidance as required resulting from such interventions. 

 

·      Organise, plan and schedule resident healthcare and allied health professional appointments.

 

·      Daily walk around overview focusing on the health and wellbeing of all residents, noting, reporting, and responding to any arising or ongoing health or care related need. 

 

·      Daily to review body maps, accident forms and incident forms. To review and document intervention, healing and appropriately conclude audit process when indicated.

 

·      To ensure audit documents and recording charts are collated, reviewed, processed, and filed appropriately and in a timely manner.  

 

·      Responsible for supporting Shift Managers/Registered Homes Manager and ensuring we are delivering the highest standards of care to our residents at all times.

 

·      Contribution to, and involvement with weekly GP contact. 

 

·      To ensure that all daily health and care audits and recording charts are maintained, kept up to date and to analyse patterns, respond to changes and ensure implementation of any measures to address arising need.

 

·      To work as part of a set shift pattern, every other weekend. 

 

·      To fully supervise the night team.

 

·      To be responsible for all clinical healthcare procedures for all residents in the home to include bowel care, PEG related activities, continence device interventions (not exhaustive)

 

·      Admissions: To work alongside senior colleagues in actioning healthcare activities and actions in preparation for admission, during admission and following admission – as directed and disseminated by Line Managers

 

·      To provide support to the wider team in practically supporting all areas of resident need and organisation of the shift as required in the event of staff cover being needed or emergency

 

Training:

 

·      Responsible for training, support and supervision of staff competency in any health related intervention carried out routinely or on a short term basis by staff members. For example (but not limited to) managing emerging skin needs, monitoring dressings, testing blood sugars, taking physical observations, catheter care

 

·      Organise and facilitate delegated healthcare training for tasks as required – ensuring that training dates and review dates are monitored. Ensuring that refreshers are organised in a timely manner and do not expire. 

 

·      Deliver medication training to identified staff members, recording competency and reviewing competency of medication administrators in a timely manner. 

 

Medication:

 

·      Monitoring healthcare supplies, ensuring that ancillary products (dressings, lubricant, household remedies etc) do not run out or expire.

 

·      Overall responsibility for maintaining the medication clinics including their cleanliness

 

·      Responsible for the monthly medication checking in and auditing process; in its entirety to include ordering, auditing, checking in, interim medication identification, returns medication process, filing and all other process elements. 

 

·      Ensure all Woodrowe generated MAR sheets are reviewed, prepared, printed, audited and filed. 

 

·      Maintain resident’s medication profiles

 

·      Maintain all elements of medication systems in line with pharmacy audit requirements.

 

 

 

Special Projects – to be agreed, amended and implemented as needed

 

Will initially include:

 

·      Infection Control - To be aware of all infection control regulations and guidance. To ensure actions disseminated from audits and inspections are implemented

-  To generate and implement care plans for all resident healthcare activities which require attention to aseptic techniques (eg stoma care and catheter care – not exhaustive)

 

·      Mouth care - To generate and implement care plans and audit tools to ensure the correct, consistent and effective delivery of mouth care

 

·      To review all oral care, care plans and action routine dental appointments within their designated review timescale. 

 

 

Reporting

·      The Healthcare Manager is responsible for reporting formally to the Registered Manager on a weekly basis.

·      To generate and deliver a monthly report on specific areas relating to the role to the Directors

An identified short term placement, usually stipulated within a time limited commissioning period; used to assess and determine needs, risks and capabilities to inform the support needs and/or ideal location for an individuals subsequent care pathway or journey. 

Ongoing Rehabilitation

 

 

A goal focused, therapeutically led placement package which is regularly reviewed both internally and by external involved and commissioning parties. Identified for specific clients who have stepped down from an acute rehabilitation setting and require a specialist service which is able to continue to develop, consolidate and achieve re-acquisition, restoration and compensation of abilities and functions in order to achieve maximum independent potential. 

Long term & Maintenance

 

 

A placement package designed to meet the needs of clients with long term, progressive or highly dependent and complex needs; which require the continued support of a specialist team in order to; prevent deterioration, maintain equilibrium, stability and quality of life. Particularly valuable to those individuals who benefit from living in an environment which is conducive and responsive to their ongoing needs and who would otherwise likely require a more clinical or hospital environment.

Short term: Respite & Emergency

 

 

An identified short term placement, usually time limited by commissioning as between 2 - 6 weeks. Woodrowe Healthcare is able to provide short term care packages for individuals who meet the home’s admission criteria for the purpose of respite care and in cases of emergency. As with all other admissions to the home any placement of this type will be subject to initial needs assessment and assessment of risk prior to any short term placement being offered.
