This role is in our 24hr Support Accommodation Service, offering housing with care and support for individuals with complex needs including mental health, learning difficulties, substance, and alcohol misuse, who may be homeless, rough sleeping living in hostels, at risk of eviction or in hospital settings.
This is a challenging but rewarding role where no two days are the same. You may be faced with challenging situations, unusual, anti-social and offending behaviours.
Our goal is to support individuals to gain the knowledge and skills they will need to maintain their mental health to reduce the risk of relapse and offending behaviours. We strive to make a difference in the lives of those we provide a service to, aiming to help them to build the confidence and skills to live as independently as they are able.

Duties and Responsibilities:
- To provide support to people with severe and enduring mental health needs with alcohol and substances misuse needs.
- To agree to abide by the Code of Professional Practice for Social Care.
- To promote individual’s independence and strive to prevent loss of independence.
- To Support individuals with money management and budgeting.
- To support individuals with maintaining their tenancy.
- Meal planning, cooking, and shopping.
- Medication administration, ordering and arranging collection/delivery.
- Attending health appointments, meetings, and reviews.
- Support with social skills, interacting with others and confidence building.
- Communication, making needs and wishes known, overcoming barriers.
- Monitoring mental and physical health, identifying triggers and sourcing intervention.
- To work with individual residents to encourage daily living skills in accordance with individual service user’s needs.
- To liaise and work in close partnerships with other professionals and agencies.
- To carry out day-to-day tasks they are unable to do for themselves.
- To develop and implement care, support, alcohol and substance recovery plans making sure they are person centred and based on individual need, wants and achievable goals.
- To be aware of service user’s risk assessments and relapse signs.
- To take responsibility and make decisions in accordance with policies, discussing with the Managers as appropriate.
- To make written reports as appropriate.
- To attend meetings and training as required.
- To keep information about the organisation, staff, and residents confidential.
- To keep up to date with the organisation policies and procedures.
- To be flexible and cover shifts when required.
- To work responsibly and safely at all times.
- To register with Social Care Wales.
We have full and part time roles available 24, 36 and 48hr contracts depending on preference. The shifts are 12 hrs. 8pm - 8am for4 waking nights.
Qualifications:
· To be in possession of or prepared to work towards QCF Diploma level 2 or 3 in Care.

We provide care for those who find it difficult to function successfully and independently in the community without additional residential support and rehabilitation.
