We specialise in assessing, caring for and rehabilitating adults aged 18-65 with brain injury and co-morbid diagnoses often related to alcohol or substance misuse, mental health, challenging behaviour, and other complex needs.

We are looking for a Rehabilitation Assistant (Level 1) to join the team.


Requirements;

  • Full time / Part time (Hours to be discussed on interview)
  • MUST be able to commit to a minimum of 30 hours per week
  • MUST have previous care experience
  • Qualification: Leve 2/3 in health and social care
  • Yes, expected to work weekends, must be fully flexible.
  • At least 2 years’ experience
  • At least 1 night per week



Job Purpose

  • The purpose of the role is to engage in the rehabilitation of Client (service user) base - predominantly adults (aged 18 to 65) with acquired or traumatic brain injury and a range of other complex needs (e.g., history of substance misuse) by:
  • Working with High View’s Trans-Disciplinary Team (TDT) to maximise Clients (service users) rehabilitation and wider wellbeing
  • Supporting individual Clients with personal and physical care to enable each, based on an assessment of their condition, abilities needs and aspirations in order to promote independence and realise potential
  • The active delivery of a planned programme of “1to1” support aimed at securing improvements in activities of daily living and maintaining quality of life as determined by Integrated Care Plans (ICP), focussing on domains including information processing and motor skills, memory, executive functioning, mood and behaviour.


Main Duties and Responsibilities

  • Be responsible and accountable for carrying out the duties and responsibilities of the post with due regard to the Company’s Policies and Procedures.
  • Champion activity in implementing ICP’s (overseen by the TDT) ensuring this is meaningful and focussed; undertaking key working as required
  • Support Clients with all aspects of their rehabilitation to achieve agreed outcomes as part of the integrated approach to wellbeing
  • Assist Clients to achieve their rehabilitation and personal goals by aiding Personal Care*1 through advice, coaching, assistance, encouragement and motivation 
  • Contribute to monitoring, reviewing and modifying ICP through maintaining accurate records and reporting (including Key Worker Reports) of Client progress against their care plan, goals and outcomes and communicate any areas of concern or significant changes in the health, social or environmental circumstances of Clients to the Line Manager & TDT.
  • Be proactively involved in everyday practice with colleagues, Clients, carers, other professionals and agencies to coordinate plans, assess risks and develop appropriate strategies
  • Ensure that medication and other health and safety protocols are followed and medication is administered according to Clients’ individual care plan
  • Support Clients in preparation of nutritious meals as part of their ICP support
  • Support Clients who are temporarily sick or at the end of life
  • Contribute to data collection for the overall purpose of monitoring, reviewing and improving the standard of service delivered by High View
  • Receive regular supervision and appraisal from High View management
  • Where required by Management provide supervision, guidance and delegation of tasks to junior staff including monitoring performance and ensuring that subordinates are clear of their roles and responsibilities

Our Facilities

Services are provided in four specially converted houses in South East London, offering 29 individual bedrooms and transitional living units for 29 younger adults. One of our houses is a female only dedicated unit.
