To work as part of a Team to deliver a high standard of care and support to all Residents and Clients. To work under the direction or delegated direction of the Home Manager to achieve the standards required by Purley Park Trust and the Care Quality Commission (CQC).

Skills, Knowledge and Qualifications
Essential
· Caring and inter-personal skills. Good communication ability (verbal and written).
· Ability to relate to and work with adults with a learning disability.
· Tact, patience, flexibility and common sense.
· No experience necessary
Desirable
· Previous experience in a caring environment.
· Health & Social Care Diploma (Level 2) (previously, NVQ2).
Shifts
· 7 am - 2.30 pm
· 2 pm - 9.30 pm
Candidates who require full-time or part-time work must be required to do early and late shifts.

Key Result Areas
· To achieve and maintain the standards of care required by the Trust and the CQC in accordance with the Health & Social Care Act 2008 (Regulated Activities) Regulations 2009 and the CQC Essential Standards of Quality and Safety.
· To achieve and maintain a high level comfort, health, safety, welfare and ensuring their care needs are monitored and met at all times.
Key Tasks
· To participate in the preparation and implementation of peoples’ Care Plans, Essential Lifestyle Plans, Risk Assessments and Health Action Plans.
· To work in accordance with the Trust’s Policies and Procedures.
· To support the People we Support to access meaningful activities to enhance their quality of life and to develop life skills as far as their abilities allow.
· To undertake all Key Worker responsibilities, including the preparation of review reports and attendance at the review meetings.
· To support the People we Support through change, including bereavement, health needs, changes in life and lifestyle.
· To assist in and support the management of those with more complex needs or challenging behaviour, following relevant guidelines and policies.
· To create and maintain a homely living environment as far as is possible.
· To work as part of a duty rota, and maintain 24-hour Staff cover at appropriate levels (weekends and Bank Holidays are regarded as normal working days). This includes a requirement to do sleeping in duties.
· To compile and maintain accurate and timely records and reports as required by the Trust and relevant legislation.
· To undertake, participate in and contribute to Staff training.
· To ensure that correct and appropriate safety measures are adhered to at all times.
· To provide First Aid as required.
· To assess, monitor and satisfy the People we Support dietary needs, including the preparation and cooking of meals.
· To undertake any other duties as may be reasonably required.
Working Relationships Support Workers must maintain close and cooperative working relationships with all other Members of Staff and professional relationships with statutory bodies (including Social Services, Health Authorities and the Care Quality Commission), recognised voluntary groups (including Befrienders and Advocates), relatives and friends of the People we Support.
