We are seeking an individual that is experienced within the care sector, specifically working with adults that have learning disabilities. This role requires the successful candidate to assess our Customer group and write accurate, detailed support plans and risk assessments. These documents will be subject to regular review and update.

The successful candidate will be able to quickly and thoroughly assess our Customer group and put their findings into our support plans and risk assessments in a manner that would allow any member of our team to fully understand how to support that Customer. We provide services for: adults with a learning disability, mental illness, physical disability and Autism.

We expect our assessor/support plan writer to really take the reins of their workflow which will initially involve the re-writing and update of all our current support plans and risk assessments, followed by the self management of the updates.


The successful candidate will have a minimum of three years experience within a senior or managerial role within the care sector. Have a diploma (QCF) equivalent qualification to Level 3 and will be able to demonstrate during the practical interview there support plan writing skills, which will be assessed on the quality of the support plan and time taken to complete the work.


You will report to the Registered Manager and your duties will include:

  • The creation of individualised, person-centred support plans and risk assessments that work towards meeting identified outcomes with the Customers, their carers and the purchasing council
  • To contribute to the development of a professional and motivated staff team via clear and detailed support plans and risk assessments
  • Additionally, any documentation within the support plan will fall within the remit of this role and whatever other documented guidance given to the staff team as necessary
  • The ability to establish and set achievable goals and outcomes with the Customer
  • To be an ambassador for the company, raising our organisations’ profile and reputation for good quality support
  • Meet all regulatory and contractual framework requirements within the creation of these documents
  • Manage all the operations associated with the creation of the documents required as part of this role
  • Highlight any training and development needs for the branch that become apparent during your assessment of Customers
  • Working with all stakeholders which include Care Managers and family members in the creation and update of all documentation that fall within the remit of this role


Entry Requirements

  • Minimum 3 years minimum experience in the Care Sector within a Senior or Manager role
  • NVQ Level 3 Required
  • Driving License
  • Computer literate


Company Systems Currently Used

  • PeoplePlanner – For all documentation uploads and compliance schedule related to documents that fall within the remit of this role
  • Gsuite – For calendar and all other documentation purposes


We are offering

  • Over £25k per annum
  • Full time work
  • Weekly or monthly pay
  • 22 pence per mile for mileage
