Care Co-Ordinator

Sisters Care Services CIC supports adults with complex health needs in their homes across Birmingham, UK. We are currently recruiting for an experienced Care Co-Ordinator to work with Sisters Care Services CIC, to deliver safe, effective, and compassionate care to services user’s with complex health needs, at their home. We are looking for an individual who is able to lead a group of teams in delivering high quality of care. An individual who can develop collaborative working relationships with other health care professionals, services, and agencies. You will also be responsible in delivering high quality patient care according to agreed Care Quality and Commission standards. You should have good written and spoken English. Should have, or willing to work towards NVQ level 5 in Health and Social Care. Also, be willing to be on call, as and when required. We want to hear from you if you have experience as a care co-ordinator, and are interested in co-ordinating, and delivering high quality care to adults with complex care needs in the UK.
