Job Role:
To be the key Administrator and Co-ordinator for Take Care and to support the Clinical Manager and Co-ordinator for Sambhana Care as required.
Place/Time of Work
Location: Mainly at the Take Care/Sambhana Care Office, 105 Hopewell Drive, Chatham, ME7 0DX
Hours of Work; 8 am-1 pm but flexible working can be agreed, Monday to Friday
Care Co-ordinator experience is preferrable but we would also consider Care assistants who want to make that step into being a coordinator.
Minimum of 2+ Years of care experience
Drivers are preferred and must be computer literate.
Key Responsibilities:
Clients
· To organise and sort out the weekly rotas for the Take Care staff to ensure smooth operation of the client visits and to encompass the client requirements
· To communicate with staff and/or clients to resolve any issues that occur when delivering the service
· To update clients on any changes to services
· In conjunction with the Care Manager to undertake regular and appropriate reviews of care plans
· To ensure care plans are updated and correct in the office and in the clients homes
· When appropriate to make ‘wellbeing’ calls to clients to ensure support. This includes following up on clients who have been unwell/discharged from hospital
· With respect to invoicing, to liaise with Timesheet manager to ensure that all information on timesheets is correct
· To assist invoicing team with invoice queries with the client when appropriate
Staff
· To organise and sort out the weekly rotas for the Take Care staff to ensure smooth operation of the client visits and to encompass any staff issues
· To communicate with staff and/or clients to resolve any issues that occur when delivering the service and to assist staff in resolving any problems
· To receive and record weekly feedback from staff via verbal feedback and timesheets and to resolve/investigate any issues that arise
· To review and approve holiday requests to ensure the business needs are met
· To plan cover for holidays and to ensure cover staff are correctly briefed
· To review and decide with Clinical Manager any cover required for statutory holidays and shutdown
· To assemble and record positive (and negative) issues to assist managers with appraisal preparation information.
· To monitor and record/investigate sick leave and to ensure that the payroll administrator and Director manage sickness absence.
· When appropriate to make ‘wellbeing’ calls to staff to ensure support. This includes following up on staff who have been unwell/discharged from hospital/suffered family issues.
· Be responsible for the reward and recognition system
· To maintain staff files/electronic filing for all staff to be up to date
· Provide cover for Sambhana Care Coordinator who is situated in the same office.
New Clients
· To support the Care Manager with assessment of new clients by home visitations and/or paperwork preparation
New Staff
· To support the Care Manager with advertising and interviewing and appointment of new staff
· To provide training support for new staff with respect to paperwork completion and passing on information regarding care support required for clients
Newsletters
· To prepare a client newsletter on a periodic basis to update the clients on company news/events
· To provide input/prepare the staff newsletter for issue on a monthly basis
